In a patient with resected adenoid cystic carcinoma of the base of tongue staged as pT3N2, grade 2, with perineural invasion and a single positive lymph node demonstrating extranodal extension (1/24 nodes), would you favor adjuvant radiation therapy alone or concurrent platinum-based chemoradiation?

Patients with adenoid cystic carcinoma should be treated postoperatively with RT alone. To date, the results of the randomized NRG study comparing RT alone with cisplatin and RT are still pending. Without data to support the use of chemotherapy, my recommendation is not to administer platinum.
