When should CAR-T be considered in the treatment paradigm for SLE?

Should it remain reserved for patients who have failed hydroxychloroquine, immunosuppressants, and two biologics (belimumab and anifrolumab), or should it be considered earlier? This question is part of our collaboration with ACR Convergence 2026 to preview a session from this year's meeting. This question is inspired by the session series: "Cellular Immunotherapy: Are We on the Cusp of a Therapeutic Revolution in Rheumatology?

It should be considered after the failure of standard-of-care (SOC) therapy, including 1 or more biologics, for the given organ involvement with a connective tissue disease (CTD).
