Are there any breast ca patients for whom you would offer PMRT if they were cN0 and ypN0?

For example, if they were triple negative or had a poor response to neoadjuvant chemotherapy in the breast?

There are a few patients, I will still offer RT for if they are cN0 and ypN0.

  1. cT4 disease
  2. T3N0- will consider for young age, triple negative

I don't routinely offer to cT1-2N0 patients unless positive margins.

Other than what @Dr. First Last said, poor response to chemo for t1 and t2 primary if aggressive phenotype (triple negative) and residual disease more than 2 cm.
