Are you more permissive of perioperative interruption of anticoagulation for VTE depending on the location and relative chronicity of the thrombus?

For patients with a recent VTE who need a procedure/surgery requiring anticoagulation interruption, what is your usual approach to bridging?

• I bridge only if anticoagulation must be interrupted within the first 4–6 weeks.
• I bridge any patient with <3 months of anticoagulation.
• I bridge only patients with high-risk VTE (e.g., LE DVT, PE), not UE or splanchnic.
• I rarely or never bridge for recent VTE.
