How would you approach a patient with newly diagnosed lupus/lupus nephritis receiving cyclophosphamide (euro lupus protocol) with active hemolysis not responding to steroids or IVIG?
Addition of rituximab on top of CYC? Increase dose of the CYC? Belimumab? Other thoughts? Consider other differentials?

What is your preferred treatment for refractory warm autoimmune hemolytic anemia with autoimmune neutropenia?
No response to steroids or rituximab. No evidence of hematologic malignancy on bone marrow biopsy.
