I would keep gentamicin for short duration until bacteremia resolved especially in patients without any kidney issues. And agree with others to use rifampin if no interactions or side effects.

There is no great evidence to support gentamicin and rifampin in staphylococcal prosthetic valve endocarditis. Academicians like me stick to evidence so I cannot support this practice.
