For patients with eGFR <30 or who are on dialysis who are starting or continuing denosumab, how often do you monitor serum calcium?

Since the 2024 boxed warning, the denosumab label calls for serum calcium weekly for the first month after each dose and monthly after that, for patients with advanced CKD. That is roughly 8 draws per 6-month cycle.

For patients on denosumab, I’ve been checking CKD labs including magnesium, and phosphorus about 2 weeks after each dose and repleting with nephrology input when levels are low. What does everyone do in practice?
