How do you approach a case of apparent worsening catatonia despite high-dose benzodiazepines and ongoing ECT?  

Could this reflect persistent catatonia, emerging negative symptoms of psychosis, or side effects from ECT or benzodiazepines? In your experience, how do you differentiate among these possibilities, and how do you decide whether to continue ECT or benzodiazepines?
