How do you handle pregnancy testing for women of childbearing age?

Do you routinely ask all women of childbearing potential to have a pregnancy test? If so, do you get a pregnancy test before their CT simulation or their radiation treatment? What age range to you test? Do you have a separate consent form addressing the risks of pregnancy and radiation?

When ordering a simulation we denote whether a pregancy test is needed. If so, it is performed before the simulation takes place.
