How do you manage HIT in a patient with high bleeding risk?

e.g. in the setting of using prophylactic heparin in the past but now requiring therapeutic anticoagulation

If bleeding risk associated with the administration of an alternative anticoagulant is unacceptably risky, I usually recommend the administration of IVIG.
