How do you manage sexual side effects in patients being treated for OCD?

Patients with OCD often require serotenergic medications (high dose SSRIs, TCAs) that carry risks of sexual side effects. Risk of relapse may be higher with discontinuing medication compared to other conditions. Common recommendations like lowering dose or switching to bupropion are often no option in this population. How do you address these side effects?
