How do you manage TE fistula formation after definitive chemoradiation for locally advanced lung cancer?

Does initial managnement always include stent placement, PEG placement and esophagus rest? Are there times when surgical intervention is warranted? Do you ever recommend hyperbaric oxygen or pentoxiphyline?
