I recommend indefinite anticoagulation for most patients with portal vein thrombosis, and at least 3-6 months if there are risk factors for bleeding. We need more information about the clinical situation of this specific patient. The presence of portal vein thrombosis extending to the mesenteric veins in a cirrhotic patient does not automatically mean anticoagulation.
