How soon do you start tucatinib for metastatic HER2+ breast cancer with CNS metastases after brain surgery?

The HER2CLIMB (Murthy et al. NEJM 2020) protocol mandated waiting 4 weeks after brain surgery. Would you initiate sooner if you felt a patient needs treatment?
