How would you approach palliation of a symptomatic cervical cancer involving the supraclavicular lymph nodes?  

Patient with stage IIB cervical cancer treated 8 years ago, now with large (>10cm), isolated, symptomatic recurrence involving the left SCL and mediastinum. Excellent performance status otherwise.

If offering palliative RT, what dose would you use for durable palliation? Would you consider using a quad shot? Would you add concurrent chemotherapy? Adjuvant chemotherapy?
