How would you manage a patient with a history of seropositive, erosive RA who is being treated for disseminated histoplasmosis and associated MAS/HLH, but is having active RA disease activity?
Patient was on infliximab and methotrexate prior histoplasmosis diagnosis. Currently on antifungal treatment and completed steroid taper for MAS/HLH months ago with clinical improvement.
