How would you manage aplastic anemia refractory to multiple agents?
Treated initially with hATG/CNI/TPO, but then relapsed. Subsequently treated with cyclosporine, danazol, ESA, and alemtuzumab, but still with pancytopenia. Not a transplant candidate due to age mid 70s.

If indeed the patient has been treated with all reasonable alternatives to BMT, the choices are 1) watch and wait with supportive care or 2) bone marrow transplantation.
