How would you treat a patient with TNBC who developed progressive disease during neoadjuvant chemotherapy with dose dense AC and weekly paclitaxel?

if the neoadjuvant therapy was sequential, e.g. Ac-Paclitaxel, and progression was not rapid/extensive, could change to the next chemotherapy regimen. In the above example I'd add carboplatin to the Paclitaxel and watch carefully. If still progressing after a few weeks would go immediately to surger...

Abraxane seems to produce responses in patients who have relapsed on paclitaxel and therefore would not hesitate to use this agent with Atezolizumab for local recurrence. Would consider adding carboplatinum to the Abraxane as well in a triple negative patient.

I would not favor capecitabine as this isn't recurrent disease. This is progressive disease while on neoadjuvant therapy.

I would try to resect if possible and in that case I would likely offer carboplatin/gemcitabine combination chemotherapy.

If this is felt to be metastatic disease and not curat...
