When would you consider withdrawing aspirin early and continuing with P2Y12 inhibitor monotherapy after PCI for ACS?

• In selected STEMI patients at elevated bleeding risk after a short course of dual antiplatelet therapy
• In any ACS patient at high bleeding risk, regardless of presentation type
• Only after a prolonged course of dual antiplatelet therapy, once ischemic risk has clearly diminished
• I would not withdraw aspirin early — I maintain dual antiplatelet therapy for the full guideline-recommended duration
