What are you posterior field borders for endometrial and cervical cancer 3DCRT plans?

Some classic references recommend the coverage of the entirety of the sacrum but the volume delineation guidelines for both seem to only cover the most proximal portion of the pre-sacral nodes (ant to S2/3 jxn). Are there references to guide this decision?
