What dose-fractionation regimen do you prefer when treating with hypofractionated whole breast radiation therapy without a boost for early stage breast cancer?

I typically use the Canadian fractionation, and boost patients who are young (<50), high grade, triple negative, or Her2+.

The new ASTRO guideline on whole breast fractionation just published in Practical Radiation Oncology (PRO) endorses the preferred fractionation schedules of 40 Gy in 15 fractions or 42.5 Gy in 16 fractions. This was a strong recommendation based on high level evidence with 100% consensus.
