What dose/fractionation regimen do you use for the moderately accelerated, hypofractionated treatment of localized prostate cancer?

There are a wide range of seemingly safe/effective regimens in the published literature, with an associated array of BED/EQDs and little more to guide one in choosing the optimal scheme.

I use 70 Gy in 28 fractions. Rationale in brief below-

  1. Tested and proven to be non-inferior in RTOG 0415
  2. Less valid but important; we have been using this regimen at the Durham VA for nearly a decade.
