What indicators have you found helpful in screening cases that would be appropriate for possible peer review in radiation oncology?

We can always find individual cases from time to time in which the care deviated from the “standard of care” and these can be referred for peer review. But have you found any general screening parameters or indicators that can trigger the possible need for peer review? Are there any radiation oncology (or oncology in general) resources addressing the peer review process?
