What is the best approach to treat a triple-negative inflammatory breast cancer with pCR following preoperative chemotherapy and a prior history of ipsilateral breast irradiation?  

The patient was treated for left breast DCIS 5 years ago to a dose of 5040 cGy with a lumpectomy boost dose of 1600 cGy with conventional fractionation

Prior to chemotherapy for her new inflammatory TNBC, she had biopsy-proven matted axillary LN that were biopsy proven
