What is the target ferritin level for patients with hereditary hemochromatosis and signs of end-organ damage?

For example, a patient with hypogonadism unless it matters which organ is involved. Are other markers of iron storage useful in guiding therapy?

I believe the best marker to guide phlebotomy therapy for iron overload is the serum ferritin concentration. I use a target ferritin level of approximately 50 ng/ml. Treatment should be initiated in C282Y homozygotes with an elevated SF, defined as >300 ng/mL in men and >200 ng/mL in women, along with a TS of ≥ 45%.

Phlebotomy should be done weekly with monitoring serum ferritin every 4 weeks till serum ferritin is <200 ng/ml.
