What is your approach to perioperative management and monitoring in a patient with acute intermittent porphyria?  

Are there any changes to transfusion guidelines or post-op follow up recommendations for these patients? How does NPO status affect AIP and surgical planning? Would you consider prophylactic management of AIP?
