What is your opinion on IORT for breast with electron brachytherapy?

We have a lot of push from our surgeons to do IORT, do you use the ASTRO APBI criteria? Do you treat off-protocol? Do you use Xoft or Intrabeam?

I have used the Intrabeam system. I use strict selection criteria, following both the TARGiT A eligibility and the ASTRO suitability criteria. I think this is a good modality for lower risk disease. Since there is a large phase III, prospective, randomized trial with long F/U (all patients beyond 5 yrs and some beyond 20 yrs) published (TARGIT-A), I use IntraBeam and follow the TARGIT-A trial eligibility criteria.
