What is your preferred therapy for MDS/MPN with significant leukocytosis and neutrophilia?

For example, WBC >400k. Would you prefer a HMA vs hydroxyurea? When would you consider for bone marrow transplant?

Depends on blast count, cytogenetics, and molecular test results, age of patient, performance status, comorbidities, etc.
