My go-to for years now has been to have my team run Lido with epi under the field with a 1-inch needle while I see other patients. I use fine forceps and Castroviejo curved scissors and superficially pinch out each les...
I usually use the erbium YAG laser with a 1 mm spot and gently ablate the lesions until most of the whitish stroma has disappeared.

Light-handed destruction with electrodessication is my go-to treatment, along with repetitive education on the genetic nature and near certainty of additional lesions in the patient’s future (no treatment is the other option, of course).
