What's your follow-up protocol for a near complete response (nCR) in rectal patients considering non-operative management (NOM)?
OPRA trial recommends continued NOM if no signs of progression in nCR patients vs. NCCN recommends repeat studies at 8 weeks, and if not converted to clinical CR, then proceed to TME.
Which recommendation do you use, and during your follow-up period, how do you address nodes when assessing response?

If the residual disease is confined to the rectum and less than 3 cm wide and less than 1 cm deep, then I would recommend local tx: contact therapy, or a full-thickness local excision.
