Which endometrial cancers do you choose to send POLE testing on, assuming you cannot check it on everyone?
If POLE testing is currently prohibitively expensive at your institution, what are the highest yield histology/grade/p53/MMRd combinations that you would choose to check it on?

This is a great question for two main reasons:
- Resource utilization
- What am I going to do with the data

Only those where it would change my treatment. Mostly high-intermediate risk, where I’d spare them VBT or not.

I hear Dr. @Dr. First Last argument. However, I am a simple person...when available, I think we should test everyone and follow the algorithm. That will minimize variability down the road if and when the patient recurs.
