I dont think we have data to suggest ALND is required in unfavorable subtypes with positive SLN if meeting up front criteria of Z011/AMAROS. Roughly 15% of Z011 were ER- and 300 of the roughly 1400 in AMAROS did not receive endocrine therapy so likely there were some unfavorable in these studies.
