Would you consider lifelong versus short-term (i.e. several months) course of anticoagulation for patients with elevated CHADSVASc score presenting with newly diagnosed paroxysmal atrial fibrillation triggered in the setting of acute illness (i.e. tachyarrhythmias with COVID-19 or bacterial pneumonia)?

Do you favor lifelong or short-term anticoagulation for patients with elevated CHADSVASc scores presenting with newly diagnosed paroxysmal atrial fibrillation triggered by an acute illness?

• Lifelong anticoagulation due to higher risk of AF recurrence and/or stroke
• Short-term anticoagulation for a minimum of 4 weeks if spontaneously converted to NSR
• Short-term anticoagulation with subsequent consideration based on individual risk assessment
• I prefer outpatient monitoring for AF recurrence with a possible implantable loop recorder to guide further anticoagulation decisions
