Do you ever initiate daptomycin plus an anti-MRSA cephalosporin as upfront combination therapy in a patient with MRSA bacteremia and an endovascular source?

I do not typically add an anti-MRSA cephalosporin in combination therapy with daptomycin up front, regardless of the source of infection. I may add that if there is a lack of clearance of bacteremia, but often this is more due to source control issues rather than antimicrobial failure, in my opinion...
