For a MALT of the eyelid, do you treat the entire conjunctiva reflection as well, or just the eyelid?   

What technique do you use (IMRT vs direct electrons w bolus)? 

The ILROG paper on extranodal NHL says "For tumors confined to the conjunctiva or eyelid, the irradiated volume shoulde include the entire conjunctival reflection to the fornices, but it need not include the entire orbit. This siutation is usually approached with a direct electron beam with bolus." However, I'm not sure why the conjunctival reflection has to be included for an eyelid MALT?
