How aggressively would you treat an incidentally noted metastasis of the orbit?  

What would you recommend for an incidentally noted enhancing lesion in the orbit adjacent to the optic nerve seen on imaging in a patient with known metastatic disease and good performance status?

Given location, would you consider IMRT to spare OARs (lacrimal gland, globe, optic nerve etc) or dose escalation (SBRT or SRS) given difficulty of re-irradiation should the lesion progress? If the lesion is asymptomatic, would observation be reasonable? 

Would your management change in a patient who is refusing biopsy of this specific lesion?
