How do you define bulky disease for Hodgkin's lymphoma which is outside the mediastinum?  

Do you use a cutoff of 10 cm? Do you measure the size of the largest node or measure the largest conglomerate or measure the total length of the entire involved chain (i.e. in a patient that has numerous involved nodes extending from axilla up to supraclavicular region)?
