If an open wound, consider getting these patients into a wound clinic for wet-to-dry dressings as appropriate (based on the size of the defect). Optimize medically (DM management in addition to smoking cessation).

I have tried a few times with mixed responses.
May try hyperbaric O2 therapy if vulvar necrosis.
