In what scenarios do you consider omitting adjuvant endocrine therapy after lumpectomy for DCIS?

Adjuvant hormone therapy prolongs DFS.

I generally do recommend hormonal therapy for DCIS. Of course, scenarios where that does not apply at those with ER/PR- DCIS or bilateral mastectomies. In addition, the recommendation in patients with DCIS s/p unilateral mastectomy is weaker and only protects against risk in the opposite breast.
