What baseline work-up or "staging" do you do in patients with a new diagnosis of Sjogren's (i.e., PFT, cryoglobulins, complement, RF, UA)?
Answer from: at Community Practice
The baseline workup I like to do in patients newly diagnosed with Sjogren's are the following lab tests with rationale:
ANA (almost all patients have a high titer ANA that is positive - typically it is a speckled pattern
Extractable nuclear antigen (ENA) testing looking for SSA (anti-Ro antibody) ...
All patients need to have some baseline labs to better understand their phenotype, which I think is essential in such a heterogeneous disease like Sjogren's. Such a workup can also be helpful in assessing prognosis, particularly the risk of lymphoma. With that in mind, I would recommend routine test...
In addition to physical examination (especially the salivary glands and lymph nodes), baseline workup for Sjogren's should include assessment of cryoglobulins and complement (particularly C4), as these are associated with worse prognosis - including lymphoproliferative disease. Less critically but p...
