When does a rising PSA after prostatectomy indicate growth just of normal prostate?   

Guidelines dont seem to account for this possibility. Could it just be normal prostate tissue growing back that is leading to PSA, why just assume it is return of the cancer particularly if the PSA rise is quite slow many years after surgery and stabilizing at a low number, ie 0.5 ng/ml?
