For any patient with alopecia areata, I recommend the National Alopecia Areata Foundation for them and their family. NAAF provides lots of resources and have a yearly patient meeting that is always a hit for patients and their families. In addition, we discuss all appropriate treatment options for t...

In my practice, I confine copy number alteration studies (SNP array testing) for cases where the biological potential of a melanocytic proliferation is in question. I do not perform the testing if the case is diagnostic histopathologically and/or immunohistochemically of a nevus or melanoma. The seco...

I would not recommend a measles vaccine for a person born before 1957. This year has been chosen because people before born before 1957 have a very very high likelihood of having had measles because virtually all children got this highly contagious disease. On the other hand, there is no harm to get...

Yes, if "family members" refers to those living in the same household or have had close contact with the affected individual within the past 3-4 weeks. Treating such contacts simultaneously is the best option to help eradicate the infestation.

Topical roflumilast is easily the most effective thing I've ever seen. Prior to that being available, here were the go-to drugs for difficult seb derm: Itraconazole 200 mg qd until better, then 200 mg qd on weekends long term Fluconazole, exact same regimen if itraconazole not accessible Oral Roflu...

First of all, it is very important not to miss underlying squamous cell carcinoma in cases labeled with erosive pustular dermatosis. I use dronabinol 2.5-10 mg qd-tid for recalcitrant pruritus (typically neuropathic or CPUO as an adjunct treatment). Very cheap using GoodRx if not covered by insurance. It’s a scheduled drug, so it requires DEA, but you don’t need a “medical marijuana license,” and it’s very well tolerated outside ...

Adjuvant radiotherapy (RT) should be considered for pleomorphic dermal sarcoma on a case-by-case basis. There is no clear data to support the use or omission of radiotherapy in this disease, but we base our clinical decision-making on three factors: Estimated local recurrence risk Morbidity of treat...

Several variables need to be explored. What "appropriate" LS therapy has been utilized and, equally important, over what duration of time for this "newly diagnosed" case? Clobetasol ointment is to date the most effective treatment, but often requires months of continuous use; prescribing physicians ...

Atypical fibroxanthoma (AFX) is a rare cutaneous neoplasm. Diagnosis is often difficult, and may require the input of an expert soft tissue or dermatologic pathologist. I have seen many of these cases diagnosed elsewhere as AFX, and upon re-review, the diagnosis is changed to cutaneous squamous cell...
