What do you start initially, and how do you go about escalating the regimen if constipation persists?

Do you commonly use prognostic tools such as PRESS to aid in your decision?

If the patient does not have underlying diabetes, do the benefits of an insulin drip for this indication outweigh the risks?

Do you extend the course of fidaxomicin past initial 10 days to at least 3-7 days after completion of antibiotics?

For example, do you routinely supplement with calcium carbonate, magnesium hydroxide, aluminum hydroxide, simethicone, sucralfate, etc.?
