How do you boost patients with squamous cell carcinoma of the cervix or upper vagina with invasion of the posterior bladder?  

Interstitial needles can result in hot spots in the bladder, but how well tolerated is that? On the other hand, intracavitary applicators usually don''t adequately cover the disease in the bladder to full dose. Or are these scenarios where external beam boosting would be appropriate?
