How do you treat endometrial cancer in the setting of a pCR after neoadjuvant chemotherapy?

Female in her 60s who had presented originally with well-differentiated endometrioid adeno clinically and radiographically involving bilateral parametria. She was felt to not be a surgical candidate upfront, and thus received 3-4 cycles chemotherapy. TAH/BSO showed no residual carcinoma. Would you recommend treatment based on initial stage? If so, would you recommend EBRT or brachytherapy?

There is no data to guide. I would favor pelvic RT based on initial stage IIIB disease. We, at our institution, usually treat these patients with neoadjuvant chemo RT followed by surgery.
