NCCN guidelines state that stage IA1 with negative margins and positive LVSI can be treated with:

radical trachelectomy + pelvic lymphadenectomy (consider sentinel LN mapping)

OR

with repeat cone biopsy or trachelectomy with pelvic lymphadenectomy (consider sentinel LN mapping).
