How do you manage unresctable thoracic esophageal cancer invading the trachea?
What chemotherapy and radiation doses/fractionation would you use once the airway has been stabilized to provide reasonably safe and effective palliation vs worsening risk for already breached airway? Any recommendation regarding stenting is also welcome.

I treat these cancers with chemoradiation therapy. That is, I don't forego treatment for fear of creating a fistula where there wasn't one before.
