This is a data free zone but several papers report a very poor prognosis for these patients. For a limited ovarian met with pelvic localized disease, one may consider chemoradiation and add adjuvant chemotherapy (to follow) but these patients generally fail systemically. Hence, chemotherapy should be given priority for these patients.

For women with a uterus, I give a combination of estrogen and progesterone therapy, even after definitive radiation therapy. Transdermal preparations have the advantage of bypassing first-pass effect of the liver, but oral combinations are also acceptable.

Early vaginal or anal cancer still has relatively high rates of lymph node involvement. In vaginal cancer, T1 lesions have lymph node involvement rates of 5 - 15%. In anal cancer, T1 lesions have a higher rate of 5 - 50%. If there are no mucosal changes then it is possible this is an in-transit LN from a burned-out primary. No data I am aware of for POLE status portending for better prognosis in non-endometrioid histologies.
For this patient, I would treat as per our institutional (and NCCN-supported) standard, which is chemotherapy + cuff brachytherapy.

The current ASCVD risk assessment calculators we have available do not contain cancer-specific parameters and thus are inadequate for accurate assessment of a cancer survivor's risk of developing CHF and ischemic heart disease. Agree with the previous comment. Current adjuvant RT for stage I and II are based on mainly histopathological classification (PORTEC-I/II). However, the recent publication of PORTEC-4a (Horeweg et al., PMID 37487144) for stage I/II showed molecular classification predicts response for stage I/II. More availability of molecular testing and further clinical experience will certainly increase its clinical usage in the future.

I think it is too early to commit all patients to induction chemo, based on what I know about this study. It is important to review the details of the study, as mentioned by Dr. @Beriwal. In addition, it is important to understand the differences between studies that led to an overall survival benefit in this study, as compared to other studies that showed less to no benefit. I would be very interested in the opinion of a rheumatologist regarding the etiology of the lytic disease in the ischial tuberosities. A decision should be made on whether to biopsy one of these lesions. My suspicion is that it is unrelated to cervical cancer, but that possibility needs to be considered.
