How would you treat an older patient with B symptoms and biopsy consistent with grade 3 follicular lymphoma but noted to have a PET scan with SUV readings of > 20?

How would a high SUV on PET influence your therapy decisions for patients with grade 3 follicular lymphoma diagnosed on adequate biopsy sample?

• No change, would proceed with standard treatment for follicular lymphoma
• Would prompt me to use more aggressive treatment regimens such as for DLBCL
• Would prompt me to re-biopsy to ensure there is no component of higher grade lymphoma, and then treat according to additional path results (if low grade, treat as FL)
